How to use the library

Please refer to the communication we have sent you, which should provide instructions on the correct document(s) to read and/or complete.

For forms to complete, please follow these steps:

• Click on the button for the form you wish to download
• Input your information on-screen into the editable PDF form, print off and sign if necessary, or;
• Print off the form, complete with a pen and sign if necessary
• Send the completed form, along with any supporting documentation requested, in a plain envelope to the following one-line address (no stamp required):
